Friends of Payton 2008 - 2009 Parent Information Form

We are trying to get to know our parents so that we may best use the resources and talents available to the school. The following information is for FRIENDS OF PAYTON (FOP) internal use ONLY and will be kept confidential within the Payton community.

For accuracy, we prefer that you fill out this online form, but if necessary you can print out a paper copy of the form and turn it into the main office.

IF POSSIBLE, WE WOULD LIKE BOTH PARENT(S)/GUARDIAN(S) TO FILL OUT THIS INFORMATION SEPARATELY. This will require completing this online form twice, once for each parent/guardian.

Information Forms should be submitted online or turned into the main office  by Monday, September 8^th.

To complete the online form, please enter your student's ID number.  Their ID number is an 8 digit number that can be found on their Payton ID, schedule, report card, etc.  If your student is an incoming freshman who is new to CPS, their ID number will be printed on the schedule they get at orientation.  If you have more than one student at Payton, please enter both ID numbers.
